Symptoms of Premature Ovarian Failure (POF)1. In individuals with undeveloped gonads, primary amenorrhea; in those with incomplete gonadal development, delayed menarche or irregular menstruation.
2. Secondary infertility due to gonadal developmental issues; menstrual cycles gradually become sparse over several years until cessation.
3. In women who have experienced menopause, typical POF symptoms include hot flashes, signs of declining gonadal function such as thirst, constipation, acne, vaginal dryness, dyspareunia, decreased libido, hypothyroidism, and urinary tract infections.
Laboratory tests:
- Karyotype: 46,XX.
- Low estrogen levels (often above 20 pg/mL).
- Elevated FSH and LH levels, with FSH rising earlier and to a higher extent than LH.
- Normal prolactin levels.
- Estrogen withdrawal test is often positive.
Ovarian ultrasound examination: If existing follicles are detected, it may indicate a lack of FSH receptors in the ovaries, leading to unresponsiveness to gonadotropins.
