What is the situation if a woman has undergone an abortion and hasn't had her period for 61 days?

Patient's question:

On October 8th, I had an abortion without anesthesia, and it has been 61 days since my last menstrual period. At 40 days, I had an ultrasound and the doctor found no abnormalities in my uterus. Yesterday, I noticed a little blood, but it wasn't significant. Last night, after having sex with my husband, I also saw a bit of blood. Today, I have bloating and pain in my abdomen. What kind of assistance do I need? How should I be treated? When can I expect my next menstrual period?

Doctor's answer:

Painless abortion for 61 days without menstruation may be due to the absence of ovulation recovery, leading to a lack of progesterone which cannot stimulate the endometrium to shed. It could also be caused by residual tissue. Additionally, it may be due to thin endometrium or uterine adhesions. If there was sexual activity post-surgery without proper contraception, it could also be a case of pregnancy again.
Generally, after a painless abortion, at least two follow-up checkups are recommended. The first should be a B scan around half a month post-surgery to check for any residual tissue. The second follow-up should be conducted after completing a normal menstrual cycle to assess the recovery of the uterine appendages.
If there is no menstruation after 35 days post-surgery, it is advisable to seek medical attention promptly.
