Abnormal leukorrhea but no leukorrhea what's going on

Patient's question:

I want to ask if I can have a gynecological internal examination even though my hymen is gone but I haven't had sexual intercourse. Last year, I had some pale yellow purulent discharge, and now my discharge is mixed with blood. Recently, I sometimes have lower back pain, and sometimes have spasmodic pain inside the vagina, which lasts for about ten seconds and then disappears. I have some bleeding outside of my period, my complexion is very yellow, and I also feel tired. I had a color Doppler ultrasound and found a cyst in the left adnexa. The careful examination of my vaginal discharge showed blood, with three plus signs of red blood cells. Can I have a gynecological internal examination of the vagina and cervix?

Doctor's answer:

Leukorrhea with blood streaks can be of physiological or pathological origin. Generally, during the ovulation period, some women may experience ovulation bleeding, also known as mid-cycle bleeding, which refers to a small amount of spotting between two normal menstrual periods and may be accompanied by mild to moderate lower abdominal pain. If the symptoms are mild, no treatment is necessary. However, if the bleeding is heavy or affects fertility, medication may be used.
However, bloody leukorrhea should also raise suspicion of the following conditions:
(1) Cervical polyps
(2) Acute vaginitis
(3) Severe cervical erosion
(4) Cervical cancer
(6) Submucosal uterine fibroids
(6) Endometrial cancer.
It is recommended to visit a gynecological clinic for examination, including a leukorrhea test, an ultrasound, and a gynecological internal examination if necessary. Once a definitive diagnosis is made, active treatment should be initiated.
