What are the symptoms of ectopic pregnancy?

Patient's question:

I had my period on February 18th, and I've been amenorrheic for 46 days. The pregnancy test showed a faint positive. On April 1st, the ultrasound couldn't detect a fetus. On April 2nd, my blood HCG level exceeded 818. There was no bleeding, and I didn't have abdominal pain, but I did have some lower back discomfort.

Doctor's answer:

Your last menstrual period was on February 18th. Therefore, by April 1st, the duration of amenorrhea is 42 days, which is equivalent to 6 weeks of pregnancy. Normal blood HCG levels at 5-6 weeks of pregnancy should exceed 850,208,003 (mIU/ml). However, you currently have a value of only 818, and the early pregnancy test shows a weak positive result. Currently, there is no visible gestational sac on the ultrasound, and ectopic pregnancy should be considered.
Although the typical clinical symptoms of ectopic pregnancy are abdominal pain and vaginal bleeding, when the blood β-HCG level is greater than or equal to 18 kU/L, vaginal MRI should be able to detect the echo of the gestational sac. If no intrauterine gestational sac is observed, ectopic pregnancy should be highly suspected.
