Is the right ovarian cyst an ectopic pregnancy?

Patient's question:

I was carefully diagnosed with pelvic inflammatory disease and a right ovarian cyst two years ago. Recently (about a week after my period), I've been experiencing frequent pain in the lower right abdomen and waist. At first, my chest also felt swollen and painful, so I went to the hospital for a thorough check-up. The doctor only suspected it might be a chocolate cyst and recommended I be hospitalized for laparoscopic surgery. But my sister said my symptoms were exactly the same as hers when she had an ectopic pregnancy. Moreover, my cyst is on the right side, but the pain lasted for five days, and starting today, the left side has also begun to ache intermittently.

Doctor's answer:

From your condition, it should not be an ectopic pregnancy, because I don't know what tests you have undergone, so I can't speculate whether it is a chocolate cyst. However, you should trust your doctor. But the risk of ectopic pregnancy is much higher than that of chocolate cysts. Ectopic pregnancy can be life-threatening, and heavy bleeding is very dangerous if the fallopian tube ruptures. Chocolate cysts are a type of endometriosis of the ovary and have a lower risk, but the risk of infertility due to endometriosis is high. Even without surgery, the risk of infertility is still high.
