Does pelvic effusion affect pregnancy?

Patient's question:

I feel my menstrual cycle is extremely abnormal: it came three times in January! The first five days—amount was extremely minimal!! On the 16th of the middle of the month, it came again, with normal flow and red blood color. But by the end of the month on the 26th, there was a slight bleeding, with very minimal amount. On the 16th, I was prescribed Bao Gong Powder at the hospital and took it for 3 days at the end of January. But throughout the whole month of February, sometimes even when I urinated, there were a few red streaks. Now at the end of the month on the 18th, it has started bleeding again. A pad is sufficient, with minimal amount. It didn’t clean up until the 28th. The blood flowing out was fresh red! On the 27th, I had an ultrasound check, and there was 31MM11MM free fluid in the pelvis. I’m really worried. What should I do now? What is going on with this?

Doctor's answer:

The greatest danger of pelvic inflammatory disease (PID) is its ability to cause infertility. The pelvic cavity of women contains the uterus, fallopian tubes, ovaries, and surrounding ligaments, including the peritoneum within the pelvis. If any part of this area experiences inflammation again, it can lead to female infertility. When acute inflammation is not treated thoroughly, it can transform into chronic PID. Additionally, if PID is not treated promptly and becomes chronic, it often results in female infertility. Since pelvic connective tissue is connected to the retroperitoneal connective tissue, which can extend up to the area around the kidneys, if acute pelvic connective tissue inflammation is left untreated, the inflammation can not only spread to adjacent organs such as the fallopian tubes and pelvic peritoneum, leading to pelvic abscess, but it can also ascend upward, causing perinephric abscess.
