Is stomach pain a sign of miscarriage?

Patient's question:

Recently my period hasn't come, and I'm questioning if I'm pregnant. Yesterday I took a pregnancy test, and it showed positive. But sometimes my stomach feels like it's cramping as if my period is coming, and I'm very worried. Could the result from the early pregnancy test be wrong? If the result from the early pregnancy test is correct, could this be a sign of a miscarriage?

Doctor's answer:

Because you have just tested positive for pregnancy with a home pregnancy test, and now you are experiencing obvious abdominal pain again, there are two possibilities. If a gestational sac has already reappeared in the uterus, which means you have been pregnant for more than 35 days, it could be a sign of threatened miscarriage. In addition to this, it could also be pain caused by an ectopic pregnancy. Another possibility is if you are still less than 35 days pregnant, this could be a sign of a biochemical pregnancy.
