Tubal blockage is primarily caused by inflammation spreading from the endometrium upwards, first leading to inflammatory changes in the fallopian tube mucosa. This results in regressive changes or patchy shedding of the fallopian tube epithelium, causing adhesion of the fallopian tube mucosa, followed by self-locking of the fallopian tube lumen or fimbriae.
