Head circumference of 345mm can you have a natural birth?

Patient's question:

I have been pregnant for 38 weeks. During my last ultrasound examination, the baby's head circumference was measured as 345. I was thinking of having a natural birth, but seeing that the baby's head circumference is quite large, can I still have a natural birth?

Doctor's answer:

The baby is now full-term, and all indicators are normal. However, whether it can be delivered vaginally depends on many factors. First, your obstetrician will measure your pelvic condition to see if the pelvic diameters are suitable for the baby's head diameter. If the pelvis is relatively narrow compared to the baby's head, vaginal delivery is not recommended. The baby is in a head-down position, but the umbilical cord is wrapped around the neck twice, which shortens the cord and makes it difficult for the baby to descend into the pelvis. This can cause resistance and may lead to fetal hypoxia. If it has been this way for a week or less, it generally has no significant impact.
