Do I still need a D&C after taking medication to expel the gestational sac?

Medical abortion, after the gestational sac is expelled, this situation usually does not require a D&C procedure. However, a small portion of patients may have issues with uterine involution, such as an overly retroverted uterus. In such cases, even after the gestational sac is expelled, our endometrium may only shed a small portion of the decidua. The shedding of decidua relies on uterine contractions. If uterine involution is not good or the uterus is overly retroverted, the decidua may not be expelled completely either. In such cases, we may use an intervention method to enhance uterine contractions or employ some traditional Chinese medicine techniques to aid in its expulsion.
After medical abortion, typically within one week of the procedure, if bleeding lasts longer than two weeks, it is important to seek follow-up care promptly. An ultrasound can be performed to check for any residual tissue inside the uterine cavity, helping to determine the next steps for resolution.
