Does a child with adenoid hypertrophy necessarily have to undergo surgery to remove it?

Patient's question:

Dear Doctor,
Hello! My daughter is 8 and a half years old. She has been diagnosed with rhinitis and adenoid hypertrophy for at least three years. The main symptoms are frequent dry nasal discharge blocking her nasal passages, which must be cleared to feel comfortable. She rarely experiences symptoms like nasal itching, sneezing, or clear nasal discharge (I am her mother, and I have the typical symptoms of allergic rhinitis).
Most of the time, her sleep is relatively normal, with steady and even breathing. In recent months, she sometimes snorts lightly when lying flat but stops when sleeping on her side. She also rarely breathes through her mouth.
Recently, she has developed a habit of repeatedly sniffing and snorting, so I had a fiberoptic examination of her nose, throat, and trachea (the report has been uploaded). The results showed severe adenoid hypertrophy, blocking more than 2/3 of her posterior nasal openings and the opening of the eustachian tube. The doctor recommended surgery.

Doctor's answer:

Good day to the parents; Adenoid hypertrophy and tonsillar hypertrophy are specific to childhood and primarily cause snoring during sleep and mouth breathing. Symptoms may worsen after a cold. If snoring persists and conservative medication is ineffective, consider a sleep study to check for hypoxia. If conservative treatment is ineffective for 2-3 months, surgery may be recommended. Your child has undergone a fiberoptic examination, showing adenoid hypertrophy (2/3), but there are no obvious symptoms, so conservative treatment is advised. Keep the nasal passages clean and use saline nasal sprays appropriately. For nasal congestion, use oxymetazoline nasal drops and apply a small amount of gentamicin ointment inside the nose after spraying. If there is no allergy or sinusitis, you can temporarily stop using budesonide. Adenoids gradually shrink around the age of 10. Strengthen your child's immunity in daily life.
