Introduction to Traditional Chinese Medicine Treatment for Optic Nerve Atrophy Combined with Retinal Bullous Detachment: Introduction: www.shouxi.et Chen Zhanrong 2004-10-8 10:57:31 Chinese Journal of Integrated Traditional and Western Medicine, August 2004, Volume 5, Issue 16 Keywords: Nerve atrophy, complications caused by excessive use of corticosteroid hormones due to optic neuritis (optic nerve atrophy combined with retinal bullous detachment), rational use of hormones, control of metabolic disorders caused by hormones. By adjusting these "disorders" to improve the immune function of the body, adopt the treatment principles of Traditional Chinese Medicine to support and strengthen the root, activate blood circulation, and regulate immunity. Targeting the disease mechanism, it achieves rapid effectiveness. The following cases are reported. 1. Case Examples Patient, male, 55 years old, visited the clinic on August 5, 2003. He complained of symptoms such as frontal headache and traction pain during eye movement for half a year, and vision deterioration. He was diagnosed with optic neuritis in two specialized hospitals and received supportive treatment: vitamins, vasodilators, energy mixtures, Ligusticum chinense, compound Salvia miltiorrhiza injection, corticosteroid hormones, but the treatment was ineffective. The condition progressed from optic neuritis to optic nerve atrophy combined with retinal bullous detachment, and he visited our clinic. Symptoms: Red tongue, yellow coating, wiry and rapid pulse, distending pain in both flanks, irritability, dry eyes, traction pain behind the eyeball, headache, dizziness, bitter mouth, and dry throat. Examination: Right eye light perception, grayish-white fundus, extensive bullous retinal detachment, grayish-white exudates in the vitreous, central foveal reflex disappeared, left eye vision 0.1, grayish-white exudates in the fundus, diffuse central foveal reflex. TCM Diagnosis: Dim vision, blue blindness. Syndrome Differentiation: (1) Distending pain in both flanks, syndrome of liver qi stagnation; (2) Grayish-white exudates in the retina, syndrome of spleen deficiency and qi weakness; (3) Bullous retinal detachment, syndrome of qi and blood deficiency. Treatment: (1) Soothe the liver and resolve stagnation (Xiao Yao San); (2) Tonify qi and nourish blood (Bu Zhong Yi Qi Tang); (3) Tonify qi and blood (Ba Zhen Tang). TCM Treatment Formula for Nourishing Blood, Harmonizing Qi, and Immune Regulation: Nourishing Blood, Harmonizing Qi, and Immune Regulation Formula (self-formulated): Poria, white Atractylodes rhizome, rhizoma Atractylodis macrocephalae, bupleurum, scutellaria, mint, platycodon, notopterygium, acanthopanax, angelica, angelica dahurica, angelica sinensis, asarum, bupleurum, astragalus, safflower, rehmannia, peony, schisandra, cinnabar, licorice. Adjust the dosage according to different conditions. After taking 10 doses, most of the exudates in the fundus were absorbed, dry eyes and traction pain behind the eyeball significantly improved. After continuing to take 20 doses for consolidation of the effect, all exudates in the fundus were absorbed, the central foveal reflex returned to normal, vision improved to 0.4 in the right eye and 0.8 in the left eye, and no recurrence was observed after a follow-up of half a year. 2. Discussion "Bullous Retinal Detachment" GASS reported five cases of "bullous retinal detachment" in 1973, characterized by multiple pigment epithelial barrier damages in the posterior pole of the retina and the formation of multiple bullous elevations. When subretinal fluid increases, it flows downward, often accompanied by retinal detachment below. Western medicine treatment lacks targeted effective drugs due to the unclear exact cause of this disease. In recent years, foreign reports have indicated that long-term use of corticosteroid drugs, such as in cases of nephritis and optic neuritis, can lead to central serous retinopathy. Some reports have even shown that the condition of central serous retinopathy patients worsens rapidly after corticosteroid treatment, even developing into multiple leaks and forming bullous retinal detachment. This disease is a complication caused by excessive use of hormones due to optic neuritis [1]. Pang Zanxiang proposed that blue blindness is often caused by yin deficiency of the liver and kidneys, qi damage due to liver qi stagnation, insufficient body fluids due to liver qi stagnation, or deficiency of the heart and spleen with liver heat. Among these, liver heat stagnation is the main cause of this disease. The pathogenesis should be discussed based on the blockage of the (microcirculatory channels), making the key of blue blindness the blockage of the and the obstruction of divine light [2]. Xiao Guoshi the cause of blue blindness is primarily due to various internal and external imbalances in the early stage and primarily due to deficiency in the later stage [3]. Wei Yuying proposed that blue blindness is often caused by internal injury due to emotions, excessive labor, congenital deficiency, dietary disorders, irregular sexual life, or prolonged illness and injury to the eyes [4]. The main changes in the pathogenesis are manifested in the four characters of stagnation, stasis, heat, and deficiency [4]. Yao Fangwei it is necessary to follow the principle of treating the root of the disease, exploring the etiology and syndrome differentiation for treatment. According to the theory that prolonged illness leads to deficiency, the principle of strengthening the root and replenishing deficiency is often adopted, and it is necessary to identify which organ or is deficient and replenish accordingly [5]. Experience and Understanding: Optic nerve atrophy is a degenerative lesion of the optic nerve. In Traditional Chinese Medicine, this disease is called blue blindness, often caused by yin deficiency of the liver and kidneys, yang deficiency of the spleen and kidneys, deficiency of qi and blood, liver qi stagnation, or stasis of qi and blood, leading to the failure of nourishing the eye oracles and the inability of divine light to manifest. The treatment principles are to nourish the liver and kidneys, warm the spleen and kidneys, tonify qi and nourish blood, soothe the liver and resolve stagnation, and activate qi and blood circulation. Based on the above theory, the author adopts the principle of treating different diseases similarly and formulates a self-formulated formula: supporting the root and strengthening the foundation, activating blood circulation, and regulating immunity. This disease belongs to the category of blue blindness, in addition to congenital deficiency and deficiency of the liver and kidneys, it is also related to high fever and blood loss, as well as overwork. Currently, there is a lack of ideal treatment methods. This formula highlights the characteristics of Traditional Chinese Medicine and is modified and adjusted from Xiao Yao San, Bu Zhong Yi Qi Tang, and Ba Zhen Tang.
