Infantile spasms are a refractory form of epilepsy encephalopathy, and the most effective treatment currently remains "corticosteroids and antiepileptic drugs." Corticosteroids are preferably treated with ACTH, which is effective in about 70% of cases, but there is a 1/3 recurrence rate after discontinuation. Commonly used antiepileptic drugs include sodium valproate and clonazepam, while newer drugs such as topiramate, levetiracetam, and lamotrigine can also be considered.
