A three-year-old child with thumb

Patient's question:

Unable to straighten, it improves after massage, but then it becomes unable to straighten again after a while. One side was treated for a year, and the other side has not been treated yet. What kind of assistance is needed: I want to heal it through my own massage. I am asking how to use many blood-activating ointments together...

Doctor's answer:

Tenosynovitis can be treated with acupoint anesthesia (1) Main acupoints: Divided into 2 groups: 1, Yangxi, Yangchi; 2, Waiguan. (2) Medicinal solution: 2 mL of prednisolone, 2% procaine (skin test must be performed first before use; if positive, use 2 mL of lidocaine instead). Choose one from each group. Select one group of acupoints on the affected side only. Draw the chosen solution into a syringe, mix well, and inject with a 6 or 5 needle directly into the acupoints. After obtaining the qi, infuse 2 mL of solution per acupoint. Treat every 2–5 days, with 3 treatments constituting one course.
Recommendation: Moxibustion (1) Main acupoints: Ashi points. Secondary acupoints: For flexor tendon tenosynovitis, add Hegu and Yuji; for radial styloid tenosynovitis, add Yangxi and Lieque. Location of Ashi points: The most tender point (press annually). (2) For miscellaneous diseases, there are two methods: one is drug-separated moxibustion, and the other is scarless direct moxibustion.
Drug-separated moxibustion: Preparation of moxibustion material: Take equal amounts of cinnamon bark (gougui) and cloves (gengying), grind them into a fine powder, and mix evenly to form Dinggui powder. Wash fresh ginger, cut it into 2 mm thick slices, and pierce 6–7 small holes in the center with a three-edged needle.
Scarless direct moxibustion method: Make pure moxa wool into wheat-grain-sized moxa cones, place them directly on the acupoints, and light them with a burning thread to extinguish the flame slowly, allowing the moxa cones to burn steadily. When the patient feels warmth, extinguish the cones with a bamboo or paper piece and place another cone on top. Repeat this process to apply 5 moxa cones in total, ensuring the warmth penetrates deeply into the affected area.
Secondary acupoints: Select 1–2 points and needle them with filiform needles. After obtaining the qi, apply equal tonification and sedation, and retain the needles until the moxibustion is complete.
