Treatment:Traditional Chinese Medicine:
For Yang jaundice, it is advisable to clear heat and resolve dampness, using modified Chen Pi Hao Tang. Basic formula: Chen Pi 15g, Fu Ling and Shan Zhi each 6–9g, Da Huang 1.5–3g, Yi Yi Ren 10g.
For Yin jaundice, it is advisable to invigorate the spleen, warm Yang, and resolve dampness, using modified Chen Pi Zhu Fu Tang. Basic formula: Chen Pi 15g, Bai Zhu and Fu Ling each 6g, Ze Xie 9g, Fu Zi 3–6g, Gan Jiang and Gan Cao each 3g; add Long Dan Cao 3g, Huang Lian and Huang Bo each 3–6g if there is dry mouth, constipation, and fever; add Chen Pi, Ban Xia, and Zhu Ru each 6–9g if there is poor appetite and nausea; add Dang Shen and Shan Yao each 6g if there is loose stools. If there is dry mouth with yellow, greasy, thick tongue coating, indicating damp-heat, it generally belongs to Yin jaundice transitioning to Yang jaundice. In this case, remove Gan Jiang and Fu Zi, and add Shi Zi, Huang Bo, Jin Yin Cao, and Che Qian Cao each 6g; add Yu Jin, Yan Hu Su, Dang Gui, Bie Jia, San Ling, and E Zhu each 3–6g if there is hepatosplenomegaly. For severe jaundice, it is advisable to clear heat and detoxify, using modified Chen Pi Hao Tang combined with Huang Lian Jie Du Tang. Basic formula: Chen Pi 15g, Shi Zi, Huang Lian, Huang Qin, and Huang Bo each 3–6g; add Da Huang 1–3g if there is severe heat and constipation; add An Gong Ni Huang Wan if there is confusion and convulsions; add Sheng Di and Shui Niu Jiao each 6g, and Dan Pi and Chi Shao each 3–6g if there is bleeding. All medications are given orally once daily, with decoction, or via nasogastric tube for those unable to take oral medication. A 7-day course is one cycle, and if the effect is not significant, a second cycle is continued.
Western Medicine:
(1) Dexamethasone at a daily dose of 0.1–0.2mg/kg body weight, added to 10% glucose for intravenous infusion or oral prednisone at 2–3mg/kg orally. After improvement, the dose is reduced. A 7-day course is one cycle, generally 1–3 cycles.
