What's the reason for a baby's farting for over 20 days

Patient's question:

Hello doctor, my baby is just over 20 days old, but her buttocks are very red and she has developed many small bumps... How should I treat it? I have applied diaper cream and purple root oil to her, but it hasn't worked after two or three days. What should I do? Thank you

Doctor's answer:

Analysis of the Condition: In medicine, baby diaper rash is also known as diaper dermatitis. The main cause is prolonged moisture and heat on the buttock skin, which can worsen diaper rash. Stimulating substances in feces and urine can also cause the buttocks to become red. Frequent contact with cleaning solutions is another contributing factor. The primary symptoms of diaper rash include redness in the diaper-covered area, sometimes with scaling or small blisters. In severe cases, it may develop into pustules, broken skin, and ulcers. Fungi and bacteria can complicate or exacerbate diaper rash. Diaper rash is not a serious skin condition, as a newborn's skin heals quickly. With proper care, it generally improves within 2-3 days. Otherwise, it can affect the baby's quality of life.
Advice:
1. Minimize the use of diapers, as this is the best way to prevent diaper rash. It is ideal to let the baby's bottom air out for several hours each day.
2. Change diapers frequently. This is also a preventive measure. Newborns urinate every 15 to 20 minutes, though their urine output is only about a tablespoon. Still, we recommend changing clean diapers every 2-3 hours, especially after bowel movements, and changing immediately afterward.
3. For severe skin breakdown, you can use a desk lamp with infrared functionality to gently warm the buttocks, but be sure to maintain a safe distance and moderate temperature. Treat for 1-2 times a day, each for 15 minutes.
4. For simple diaper dermatitis (non-infected), apply diaper cream, ointment, or other baby skincare products. For infected cases, use antifungal creams like Daktar or clotrimazole.
5. When washing diapers, avoid harsh irritants like soap or laundry detergent. For urine-soaked diapers, boil them and then rinse with clean water. For feces-soaked diapers, use mild, non-alkaline items like mild soap, then dry them in the sun.
