Patient's question:
Age description: Suddenly unable to see clearly with the right eye, but after more than ten minutes, vision returned, but there was a black shadow below when looking at things. After checking at the hospital, the doctor did not find a retinal break but suspected bleeding.Doctor's answer:
. Anticoagulant therapy, in addition to etiological treatment, requires the use of anticoagulants such as heparin and coumadin, which inhibit the formation of prothrombin. Daily monitoring of prothrombin time is necessary to prevent the risk of systemic bleeding. Alternatively, fibrinolytic enzymes can be used.2. Integrated traditional Chinese and Western medicine treatment includes oral administration of vitamin C, rutin, and vasodilators. Simultaneously, traditional Chinese medicine treatment is provided: in the early stage, the focus is on clearing heat and cooling blood, combined with invigorating blood circulation and resolving stasis; in the middle stage, the emphasis shifts to invigorating blood circulation and resolving stasis, with supplementary clearing of heat and brightening eyesight; in the late stage, the treatment aims to nourish the liver and kidneys, replenish qi, and brighten eyesight. The effectiveness of this pharmacological approach remains to be evaluated.
3. Laser treatment for retinal vein occlusion has two objectives: 1) treating chronic macular cystoid edema, and 2) destroying non-perfused capillary areas to reduce the formation of new vessels.
4. Surgical treatment is necessary when venous thrombosis leads to complications such as vitreous hemorrhage or retinal detachment. Treatment principles:
① For cases that show no improvement after more than 4 months of pharmacological treatment, argon or krypton ion laser irradiation can be employed.
② Using low-energy laser, irradiate evenly and sparsely along both sides of the obstructed branches in hypoxic areas, with a laser spot size of 200 μ and spacing of about one laser spot. The treatment should be completed in 2–3 sessions within 2 weeks.
③ Laser coagulation points should be avoided from major blood vessels, the macular area, and the disc-papillary bundle.
